| Child Enrollment Application | -The initial form needed to be filled out to enroll your child in our day care center. |
| Medication Permission Form | -This form gives us information on medication that your child is to be given. |
| Medical Authorization Form | -This form gives us authorization to take immediate action in a medical emergency |
| Alternative Pickup Form | -If someone is to pick up your child other than yourself, this form will need filled out. |
| Emergency Contact Form | -This form needs to include all emergency numbers and alternative contacts. |
This is an optional webpage. We have included it as an example of a general topic that may be desirable for use in your own website. A page like this would be customized with your own text, graphics and documents if it were in use for your own Child Care Center.
This is not a ‘canned’ or ‘cookie cutter’ website. We custom design and build every Child Care Center’s web page from scratch. This entire website is only for demonstration purposes.
